‘I Couldn’t Even Open My Eyes. That’s The Extent of the Swelling I Was’

Onemanda Preisinger is nervous about her child’s impending 13th birthday party. It’s not due to the usual reasons associated with a home packed of clamorous preteen children, but because it’s the first time she will debut her recently altered face to acquaintances and extended family. “Obviously I’m going to tell all guests as they arrive, ‘For your information, this is not the way I appear,’” says the 30-year-old real estate agent from Southern Florida.

How she looks is, well, a little startling – her face swollen and preternaturally lifted, as though secured by industrial-grade tape. Her new – and she’s keen to stress, temporary – appearance is the outcome of half a dozen cosmetic procedures, such as an endoscopic mid-facelift, performed by a surgeon in Turkey’s Istanbul, the previous month. “My spouse became emotional when he viewed me for the first time because I wasn’t able to even open my eyes. That’s how puffy I was,” she tells me via video call from her home. “I had to tell him: ‘Honey, I’m okay, I’m not hurting. I just appear as if someone jumped me.’”

Growing Trend of Early Facelifts

According to plastic surgeons, Preisinger is one of a growing number of individuals choosing to have a facelift in their twenties and thirties – well over a decade before typical surgeons’ usual candidate age of forty to sixty. (Though most surgeons are eager to highlight the industry edict of: “We address heredity, not age.”) “I have individuals in their late twenties requesting facial lifts,” states London-based cosmetic surgeon Georgios Orfaniotis, a ex-NHS doctor who focuses on the facial area. “It’s a very significant procedure, and I’m a somewhat worried when I see people opting for it as a lifestyle choice.”

Explaining the Rhytidectomy Surgery

A rhytidectomy, alternatively called a facelift, elevates the ligaments in the face that start to sag as we age. “Human faces are structured a bit like layers of an onion,” explains Kent-based face specialist Marc Pacifico. “Dermis and adipose on the top, then a layer of connective tissue called the superficial musculoaponeurotic system. Think of the facial framework as the structural foundation of the face. And that is what causes aging; that is what becomes lax and drags the skin down with it.”

You risk performing surgery on individuals that have underlying problems. It’s not a practice for an ethical plastic surgeon to pursue

Once the preserve of moneyed older people, “filler fatigue” – when excessive use of injectable gels expands the face and causes looseness in the skin – combined with the widespread use of weight-loss drugs, low-cost surgical travel, and the advancement of novel, celebrity-endorsed surgical techniques are driving a different type of patient towards this invasive surgery. Reports show an eight percent rise in facelifts over the past 12 months in the United Kingdom; while a study found that the portion of youthful candidates is growing, with 32% of facelifts now conducted for patients aged 35-55.

“People are now requesting to look like the best version of themselves and obviously the methods are evolving,” says Orfaniotis. Currently, the trending procedure is the comprehensive facial lift, a method evangelised by everyone from Kardashian matriarch Kris Jenner, to style creator Marc Jacobs.

Advanced Lifting Method

Where a more traditional facelift entails elevating the SMAS, the deep plane facelift loosens the facial ligaments underneath, enabling doctors to reshape the face by moving the underlying layers, and avoiding the tight, stretched look occasionally caused by older methods. “We avoid placing tension on the skin because we’re targeting deeper, to the deeper tissue,” says prestigious London cosmetic doctor Rajiv Grover, who focuses on the deep plane technique. “It is the underlying layers that is adjusted. The skin comes with it as a secondary element.” Elevating the entire soft tissue layer as a unified block allows for a more natural-looking and more durable result. It also means that the surgery is not just being used for the express purpose of anti-ageing – or rejuvenation, as the aesthetics industry describes it – but for “beautification” and contouring, too. “We receive numerous requests from clients aged 28 to 35 for this purpose,” says Grover.

Individual Experience

Preisinger had never planned on undergoing a facelift – at minimum not at the thirty years old. But long-term using injectable gels in an effort at “symmetry correction” meant that by the time she reached her late twenties, she was “botched”. “I didn’t undergo this because I wanted to look young,” she clarifies. “I underwent it because the injectable harmed my face. I regret ever done it. If I didn’t have the injectable, I don’t believe I would be in this situation right now.”

If dermal fillers completely disappear has historically been a debated topic in the aesthetics industry. “Studies have indicated that they seldom completely dissolve,” says Grover, “but they move and can block drainage pathways. One of the contributors to what we call ‘puffy appearance’ is additionally the reality that to some extent, the face gets slightly fluid-filled because its capacity to drain bodily fluid has been reduced.” Though research into the topic is early-stage, cautionary statements on dermal fillers’ potential impact on the body’s drainage have been issued, and further research announced. One highly regarded face specialist, speaking anonymously, mentioned he would avoid using injectables in a patient because of the risks they present. “Many doctors avoid talk about this, because the companies who make filler can be pretty aggressive.” He’s been told accounts, he says, of plastic surgeons facing legal actions after raising their concerns.

Decision and Procedure

For Preisinger, once she started injecting filler, she felt as if she had to keep adding more – especially as it began moving to different areas of her face. She was only twenty-eight when a nearby cosmetic doctor in the state proposed that a facial and neck surgery might be what she required to finally step off the injectable hamster wheel. Two years later, she found herself selecting from a selection of suggested accommodations in the city, texted to her by the clinic assistant at her doctor’s clinic.

Initially, Preisinger had planned for an upper and lower blepharoplasty – better known as eyelid surgery – but her doctor recommended that a facelift was the correct option for the laxity she was experiencing in her face. She booked a brow lift, a mid-facelift and a jawline surgery. The eve of her operation, 24 hours post she’d touched down alone in the country, she decided to add a lip lift. “Then he said, ‘We will elevate the under-eye bags.’ He added, ‘I think if we extract some cheek fat, it will sculpt your face.’ So I ended up adding two more procedures,” she says. She paid $22,000 (£16,500) for the half-day surgery, including a three-day|
